What Defines Insanity?

A quick thought; when a man declares he is Napoleon Bonapart we send the men with the nets and straight jackets to cart him away. He is possibly a damger to himself or others. But when a man says he is a 6 year old girl we extend legal protections to him and let him go to the bathroom with real 6 year old girls, lest we damage his self-esteem.
What's wrong with this picture?
